Send the completed and signed Fraud Protection Claim Form with -
1. A copy of the end of auction notice or a copy of the auction page
2. Proof of payment, ie. a photocopy of the front and back of money order, cashiers check, personal check, copy of PayPal, PayDirect or Billpoint payment statement.
3. If you paid via credit card, also include the denial of reimbursement from your credit card company (you need to request this from them if they don't send it automatically)
4. Any other supporting documentation
